This is a drive adapter, Wright Tool 2413, in 1/4-inch drive, 1/4 inch, 1-11/16 inch long. Wright hot-forges its tools from alloy steel at its own Ohio plant rather than buying forgings in, and the working faces are cut to bear on the flats of the fastener rather than the corners.
